Main duties of the job
Clinical:
· Participation in daily preparation of repeat prescriptions to ensure high quality, safe and timely repeat prescribing.
· To perform synchronisation of medication supplies; perform amendments to issue durations of prescriptions to match actual medication use duration; co-ordinate and lead efforts to satisfy medication monitoring requirements; liaise with the administrative team about patient discharge information and clinical letters to perform medication reconciliation
· Respond to medication queries from patients and staff, take appropriate action
· Conducting medication reviews and reconciliation post-hospital discharge.
· Working with the clinical pharmacist and GPs to optimise prescribing.
· Providing advice to patients and staff on safe and effective medicine use.
Systems
· Provide advice to GP's, staff and patients in changes to prescribing, to support the improvement of prescribing safety, quality and cost effectiveness.
· Participate in medication audits and Quality Improvement Projects.
· Plan, develop and support the introduction of new working processes to optimise the quality of prescribing.
Communication and Collaboration
· Liaise with GPs, pharmacists, nurses, and local pharmacies to resolve medication issues and ensure safe transitions of care
· Maintain professional and sensitive communication with patients, considering accessibility and consent
· Contribute to shared learning and consistency across PCN practices.
Audit, Records and Compliance
· Maintain accurate clinical records on EMIS
· Support audits, data collection, and performance reporting
· Uphold confidentiality, data protection, safeguarding, and infection control standards.
Learning and Development
· Maintain GPhC registration and CPD portfolio.
· Engage in supervision, appraisal, and quality improvement activities.
· Participate in staff training, clinical governance, and multidisciplinary education sessions.
General Duties
· Follow policies and procedures, manage workload effectively, and report incidents or complaints promptly.
· Promote equality, diversity, and inclusion in all aspects of work.
· Adhere to health and safety standards and uphold the professional image of the PCN.
Qualifications/experience
· BTEC or NHQ level 3 in Pharmacy Services or equivalent
· Current registration with the General Pharmaceutical Council
· Previous GP Practice experience is desirable
· Good Clinical knowledge
· Excellent communication, interpersonal, and organisational skills.
· The commitment to continue to learn and develop.
· A proactive approach with a strong focus on patient safety and care.
· Competent in using clinical systems and confident with IT.
· Support care home residents with reconciliation, synchronisation, safety training, and medication changes.
· The confidence to work as part of a busy GP practice
· The ability to adapt to be able to convey information to patients and fellow healthcare professionals.
